PET GROOMING RELEASE FORM

Thank you for choosing Muddy Paws Mobile Grooming Salon for your pet’s grooming needs.  Muddy Paws prides itself on providing the best care for your best friend.                                        

There are some policies Muddy Paws would like to inform you, the owner, of before grooming your pet:

 1) Sometimes grooming can expose or aggravate a hidden pre-existing condition.  I cannot diagnose a condition, but may advise the owner to seek veterinary attention.   If your pet has any medical problems, Muddy Paws asks to be made aware of them (i.e. seizures, arthritis, collapsing trachea, etc.) so that the groomer may take whatever steps needed to keep your pet comfortable.

2)  If fleas are found on your pet(s), it is Muddy Paws’ policy to give a flea bath automatically.  We will try to call or notify you if possible.  There is an extra charge for this service to be determined by how much time is involved removing the fleas from your pet and our salon. This cost covers the special shampoo, time and extra clean-up to ensure no fleas are active in the salon. Flea shampoo kills the fleas on the pet.  If fleas are in the home or pet area, the flea shampoo will not protect the pet(s) from them once they leave our salon. A Capstar pill may be administered under extreme cases, with your permission.

3)  If your pet has any history of biting, Muddy Paws requires notification of this.  This will help prevent injury to the groomer and your pet(s).   If your pet has behavior issues, Muddy Paws will be happy to do what we can to help work through them.

4)  It has been our experience that when a pet is “shaved down”, pre-existing conditions may appear.  When a pet is very matted, air cannot get through to its skin.  If the pet has an itch and bites itself, the skin will become sore and will not be able to heal through its matted coat.  Shaving uncovers these skin conditions, as well as other skin problems that may have developed from the lack of air and grooming.  Please note that it is difficult to shave down a matted pet without slightly irritating the skin.  THE ONLY WAY WE WILL CONSIDER SHAVING YOUR PET WILL BE WITH YOUR CONSENT.  There will be an extra charge for this service because of time and additional blades needed.

5)  Cancellations happen and we understand schedules sometimes change.  If you must cancel we ask that you give as much notice as possible.  Chronic cancellations may result in the inability to book future appointments and a cancellation fee up to the amount of the grooming.
 No shows.  If we arrive and find no one home you may be subject to a trip fee.  Habitual no shows may be subject to a fee up to the price of the grooming and may no longer be able to schedule future appointments.

There is a $25 fee for returned checks.

6)  Muddy Paws Mobile Dog Grooming reserves the right to refuse service to customers whose pets may pose a threat to our employees and other pets left in our care, whether it is aggression problems, health problems or parasite problems other than fleas.

7)  Although accidents are very rare, there is a risk when dealing with pets and sharp objects.  Muddy Paws Mobile Grooming Salon will inform you immediately of any incident that occurs or that we notice. Your pet(s) safety and comfort is our number one priority.

 I further understand and agree to release and hold harmless Muddy Paws Mobile Grooming Salon, its owner, employees and affiliates from and against any and all liabilities, expenses, damages, and costs, including reasonable attorney fees, resulting from any service provided or injury including death to your pet(s) whether in our care or after our services have been completed.  If necessary we will transport your pet to a veterinarian.  Any veterinary bills resulting from a pet being matted, senior, or from a pre-existing or unforeseeable condition will be the sole responsibility of the owner.  Safety is our number one concern.
